The deadline to submit a Certification Form is 11:59 p.m. EST on March 25, 2026.
Considerations and Preparation:
To be considered for eligibility for a Distribution Payment from the Unikrn Fair Fund, you must provide the information requested in this online Certification Form. Failure to submit the completed form and any required documentation through the Fund Administrator’s online claim system so that it is submitted and received on or before 11:59 p.m. Eastern Standard Time on the March 25, 2026. Certification Bar Date set forth in the Plan may result in your claim being rejected and you being precluded from any recovery from the Unikrn Fair Fund.
Timely submission of your completed Certification Form does not guarantee you will be eligible for a Distribution Payment from the Unikrn Fair Fund or that you will be compensated for your claimed loss.
Please fill out this Certification Form completely. If you fail to provide all relevant information, the Fund Administrator may reach out to you to request additional documentation to support your claim. Failure to completely fill out the Certification Form or timely provide all requested documentation will result in rejection of your claim.
Providing a valid email address is required as part of the Certification Form. Failure to provide a valid email address that remains valid throughout the administration of the matter may result in rejection of your claim. If you must change your email or any other contact information, please contact the Fund Administrator and request a secure link to transfer your updated information.

This Certification Form requires you to provide information required to substantiate your control over a digital or physical wallet. If, in the discretion of the Fund Administrator, a Preliminary Claimant fails to prove control over a digital or physical wallet within the allotted time he, she or it will be deemed ineligible to participate in the distribution of the Fair Fund. The Fund Administrator will have the right to request any additional information and/or documentation, including information supporting complicated transaction histories or involving certain cryptocurrency exchanges. Persons who fail to timely provide additional information as requested will be deemed ineligible.

If you are not a U.S. Person, you must asubmit a completed IRS Form W-8BEN, W-8BEN-E, or other W-8 series form, which can be found by visiting the following IRS website: www.irs.gov/forms-instructions.
Submitting your Certification Form using this Fund Administrator’s online claim system, which includes a digital wallet connectors designed to establish proof of your control over the wallet, allows you to forgo the manual entry of your claimed transactions as part of a paper Certification Form. Online submissions may also result in faster review and consideration of your claim. To start your online Certification Form, you must login with your User ID and PIN. You can find your User ID and PIN on the Plan Notice you received. If you are unable to locate your ID & PIN, please contact the Fund Administrator by emailing info@UnikrnSECFairFund.com.
